Wyoming Central Visual Acuity Disability Permit

Wyoming Game and Fish issues a Central Visual Acuity Disability Permit to hunters whose vision permanently does not exceed 20/200 in the better eye with corrective lenses, or whose visual field is no wider than 20 degrees. The two-page application has the hunter's sworn statement and an eye or medical professional's certification on page 1 and a proof of residency form on page 2. Holders also qualify for a separately issued Disabled Hunter Permit.

Who completes it

The hunter completes the personal section, including Social Security number, and swears to being permanently disabled; Wyoming residents also give a Wyoming driver's license or ID number, or complete the residency form. A licensed physician, optometrist or ophthalmologist completes the certification with name, address, phone, signature and date.

Submitting and payment

How to submit it
Permits are issued at Wyoming Game and Fish regional offices or headquarters in Cheyenne, and the application can be mailed to the department's License Section.
Who pays
The application lists no fee for the permit or for the professional's certification.

Good to know

  • The certification is sworn: under Commission Chapter 35, a clinician who makes a false statement so someone can obtain the permit violates the regulation and faces Title 23 penalties.
  • The qualifying vision must be permanent; the form has no temporary option.
  • Residents without a current Wyoming license or ID must prove one year of residence, Wyoming school attendance or qualifying military status on page 2, or the license is denied.
  • The current version is revised 8/2023. A central visual acuity disability also qualifies for Wyoming's hunting season extension permit.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
