California DFW355C: Free Fishing License, Mobility

California residents so severely physically disabled that they are permanently unable to move from place to place without an assistive mobility device qualify for a free sport fishing license under Fish and Game Code Section 7151(a)(4). A licensed physician completes the physician section of DFW355C and attaches a certification letter on medical office letterhead using the wording CDFW specifies.

Who completes it

The applicant completes page 1 and authorizes the physician to verify eligibility. A licensed physician completes the page 2 section and signs under penalty of perjury; a nurse practitioner or physician assistant cannot certify in place of a physician.

Submitting and payment

How to submit it
The applicant mails the application, the physician's letter and a copy of state-issued ID to CDFW's License and Revenue Branch, or uploads them through a secure FTP link requested from CDFW; allow 15 business days.
Who pays
The license is free; additional validations or report cards are purchased at the regular fee.

Frequently asked questions

Can a nurse practitioner sign California's free fishing license for mobility impairment?

No. DFW355C states that a nurse practitioner or physician assistant cannot certify the application in place of a licensed physician. The licensed physician completes the page 2 section and attaches a letter on medical office letterhead naming the mobility device or devices the applicant relies on.

Does a returning angler need a new physician letter every year?

Not necessarily. CDFW's disabled angler page says verification by a licensed physician or a copy of the previous year's free fishing license is required, so a prior free license can stand in. After the first license, renewals can be handled through license agents, select CDFW offices or online.

What wording does CDFW require in the physician's letter?

The letter, on medical office letterhead and signed by the physician, must state that the applicant is so severely physically disabled as to be permanently unable to move from place to place without the aid of the named assistive device or devices, such as a wheelchair.
