PA Game Commission PGC-707-LE Vehicle Blind Permit

Under 34 Pa.C.S. section 2923, the Pennsylvania Game Commission issues a Disabled Person Permit that lets a licensed hunter sit in or on a stationary vehicle with a loaded firearm and shoot at game. The packet includes an applicant notice, a physician's notice and the two-page application. The Commission decides each case from the physician's yes/no answers, test results and narratives.

Who completes it

The applicant completes Section I only and authorizes release of medical records to the Commission. A licensed physician completes Section II for permanent conditions or Section III for temporary ones, plus the physician certification; chiropractors may certify mobility only, not lung or cardiovascular disease.

Submitting and payment

How to submit it
The application is mailed by first-class mail to the Commission's Bureau of Wildlife Protection in Harrisburg; it is not accepted in person. Allow at least 8 weeks, longer from August through November.
Who pays
The packet sets no fee for the permit or for the physician's certification.

Good to know

  • Mobility category A.1 requires a permanent condition and inability to walk without a wheelchair, walker, qualifying braces or prostheses, or two crutches or canes; one cane does not qualify.
  • Lung disease needs FEV1 under one liter, PaO2 under 60 mm Hg on room air at rest, or 24-hour oxygen; cardiovascular disease needs AHA Class 3 or 4 limits.
  • Temporary permits cover leg, hip or back casts or surgery before maximal medical improvement and expire June 30 each year; broken arms, ribs or clavicles do not qualify.
  • Pain or fatigue alone does not qualify, and a disability parking permit or another state's permit does not automatically qualify. Refusing to discuss the case gets the application returned.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a chiropractor sign Pennsylvania's PGC-707-LE?

Only for mobility. The physician's notice says licensed chiropractors may complete the application as to mobility but not as to lung disease or cardiovascular disease, and should refer patients to the appropriate physician for those conditions. All other sections must be completed by a licensed physician.

Should I complete Section II or Section III of PGC-707-LE?

Section II is for permanent conditions where the patient has reached maximal medical improvement, including progressive diseases and old injuries. Section III is for temporary conditions, generally expected to recover within a year. Complete both only for two or more unrelated conditions, explained by narrative, never for one condition.

Why is a PGC-707-LE application returned?

Unanswered yes/no questions, missing narratives or test dates, incomplete or illegible applicant information, or a physician unwilling to discuss the patient's condition with the Commission all lead to a returned application. Returned applications can be resubmitted with the missing information or corrections.
