Idaho Fish & Game Permanent Disability Certification

Idaho residents with a permanent disability can buy a Disabled Combination hunting and fishing license or a Disabled Fishing license at a reduced fee. One way to qualify is this certification, in which a physician, physician assistant or nurse practitioner confirms a mobility or vision impairment that has no expectation of fundamental or marked change at any time in the future.

Who completes it

The applicant completes the personal section, including the last four digits of the SSN, and signs. A physician, PA or NP licensed in the United States or Canada completes and signs the Doctor/P.A./N.P. Certification.

Submitting and payment

How to submit it
The applicant returns the form with the license issuance fee to any Idaho Fish and Game office.
Who pays
The applicant pays the reduced disabled license fee; the form sets no fee for the certifying practitioner.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does permanent disability mean on Idaho's certification form?

Idaho rule defines it as a medically determinable physical impairment that a physician has certified has no expectation for a fundamental or marked change at any time in the future. The practitioner certifies that standard along with the specific mobility or vision criterion the applicant meets.

Does the patient need a new certification every year?

No. Under Idaho Code 36-406(g), once the department has a permanent disability determination on file, it stays in the electronic filing system and the license holder does not have to present a physician's determination or prove the disability each year.

Can the applicant qualify without this medical certification?

Yes. Idaho also accepts a Social Security benefit letter showing current SSI or SSDI, a Railroad Retirement Board disability letter, a VA letter showing a service-connected rating of 40% or more or a nonservice-connected pension, or an Idaho driver's license marked as disabled.
