Oregon ODFW Disabilities Hunting and Fishing Permit

Oregon's Department of Fish and Wildlife issues a Disabilities Hunting and Fishing Permit that lets a qualifying person hunt from a parked vehicle off the roadway, have a companion finish a wounded animal, and get help angling or digging clams. The 12-page packet includes the rules and the application. The permit is valid for five calendar years and expires December 31 of the fifth year.

Who completes it

The applicant completes the personal information page and signs the affidavit on the last page. A licensed physician, licensed physician assistant or certified nurse practitioner completes only the criteria pages that apply, initials each qualifying condition, describes it, and signs the practitioner certification with license number.

Submitting and payment

How to submit it
The signed application is mailed or faxed to ODFW License Services in Salem. ODFW asks for it at least 30 days before the hunt's tag sale deadline.
Who pays
The packet lists no fee for the permit itself; hunting and fishing licenses and tags are bought at regular rates, and no fee is set for the practitioner's certification.

Good to know

  • Qualifying conditions under ORS 496.018 are full-time reliance on a mobility aid, severe lung disease, AHA Class III or IV cardiac limits, inability to hold a firearm, bow or rod, or 20/200 vision.
  • For each condition, the practitioner must explain in layman's terms how the impairment would appear to a law enforcement officer, and state it if the impairment is not visible.
  • Lung disease qualifies at an FEV1 below 35% of predicted or arterial oxygen tension below 55 mm Hg on room air at rest.
  • Veterans with a VA or Armed Forces rating of at least 65% may send that certification instead of the practitioner pages.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify Oregon's Disabilities Hunting and Fishing Permit?

A licensed physician, licensed physician assistant or certified nurse practitioner. The practitioner initials yes or no on each qualifying criterion that applies, leaves the others blank, describes the impairment in plain language, and signs a sworn certification with license number. Disabled veterans rated at least 65% can use VA documentation instead.

How long is an Oregon disabilities permit valid?

Five calendar years; it expires on December 31 of the fifth year. Applicants filing after November can ask for the permit to start in January. The permit is not a license or tag, so the holder must still buy the regular hunting or fishing license and tags.

Why do Oregon disability permit applications get delayed?

ODFW says most processing delays come from incomplete answers, and incomplete applications are not accepted. The practitioner must initial the qualifying condition and explain how the disability affects hunting or fishing. ODFW may ask for more documentation or a phone interview, so submit 30 days before the tag deadline.
