Oregon medical forms for physicians, NPs and PAs

Workers' comp reports, Paid Leave Oregon verifications, DMV placard and driver forms, OHP prior authorization, POLST and Death with Dignity forms, by program.

Oregon's forms come from the Workers' Compensation Division, Paid Leave Oregon, the DMV, the Oregon Health Authority and the Division of Financial Regulation. Clinicians meet these most: the Worker's and Health Care Provider's Report (Form 827) and Return-to-Work Status (3245), the Verification of Serious Health Condition Form (PL 811), the Uniform Prior Authorization Prescription Request Form (440-4992), the disabled parking placard application (735-265) and the Oregon POLST. This page lists all 46 Oregon forms, grouped by program.

Send Form 827 to the insurer within 72 hours of the first visit. NPs, PAs, chiropractors and naturopaths must file a WCD certification statement before acting as attending provider. The PL 811 may not be signed more than 60 days before leave starts, and Paid Leave Oregon also accepts BOLI's OFLA and FMLA certification. The signer's office submits each POLST to the Oregon POLST Registry unless the patient opts out. Spanish versions exist for 6 of these forms.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is Oregon workers' comp Form 827 due?

The provider sends it to the insurer within 72 hours of the first visit, with a copy to the worker. Form 827 covers the first report of injury or disease, aggravation claims, new or omitted condition requests, a change of attending physician or NP, progress and closing reports, and palliative care requests. The worker completes the worker section for claims and change requests. It comes in English and Spanish.

Who can sign the Paid Leave Oregon PL 811?

An authorized health care provider under Paid Leave Oregon's rules, including a physician, physician associate, NP, nurse-midwife NP, direct-entry midwife, RN, psychologist, regulated social worker, optometrist, dentist, naturopathic physician or chiropractic physician within limits. Complete Parts C and D. Do not sign more than 60 days before leave starts, and do not use strikeouts or whiteout. The claimant uploads it to Frances Online or mails or faxes it.

Do Oregon payers have to accept a uniform prior authorization form?

Yes, for prescription drugs. Under Oregon's administrative rules, payers must accept the Uniform Prior Authorization Prescription Request Form (440-4992) for prescription drug requests. The prescriber completes urgency, member, prescriber, medication, previous drugs tried and medical rationale, then signs. Send it to the plan named at the top by fax, email or portal. Oregon Health Plan fee-for-service members use OHP 3978 instead.

How does an Oregon POLST get to the registry?

The signer's office submits the signed form to the Oregon POLST Registry, which is required unless the patient opts out. An MD, DO, NP, PA or ND completes the orders in Sections A to C with the patient or representative and signs Section E. Official pink forms are ordered from the Oregon POLST Program; only a patient-education sample can be downloaded. A signed POLST must be in English.
