Oregon OHP 3155 Standing Positioner Justification

Oregon Health Plan uses this justification when a fee-for-service member needs a standing positioner, or stander, approved through prior authorization. The treating physical or occupational therapist and the prescribing practitioner document the clinical reasons for the device. It is attached to the MSC 3971 prior authorization request that OHA reviews.

Who completes it

The treating physical or occupational therapist completes the clinical justification with the prescribing practitioner. The requesting provider files it with the prior authorization request.

Submitting and payment

How to submit it
Attach it to a prior authorization request in the OHA Provider Portal or on the MSC 3971. For a request already on file, add it in the portal by PA number or fax a new MSC 3971 listing that PA number on the MSC 3970 coversheet.

Good to know

  • Without the existing PA number on the MSC 3970 coversheet, OHA processes faxed documents as a new request, which delays review.
  • Only fee-for-service standers go through this OHA process; a coordinated care organization member's stander follows the CCO's procedures.
  • Send only the clinical documentation the review needs; OHA notes that extra documents may slow the decision.
  • All providers on the request, requesting, performing and referring, must be enrolled with the Oregon Health Plan.
