Alaska Medicaid General Prior Authorization Form

Alaska Medicaid prescribers use this two-page form to request prior authorization for a medication that lacks its own drug-specific form, or to exceed the maximum allowed units. It collects member, prescriber, pharmacy and drug details, then diagnoses, current medications and medical justification, including previous failed therapies with dates. Prime Therapeutics reviews pharmacy PA requests for the Alaska Department of Health.

Who completes it

The prescriber completes the whole form and signs the required attestation that the treatment is indicated, necessary and meets Alaska Medicaid guidelines. A requestor name and title can be added when staff prepare the request.

Submitting and payment

How to submit it
Fax the completed form to Prime Therapeutics, or submit it electronically through CoverMyMeds ePA.

Good to know

  • Incomplete requests are denied until all required information is received, and approval does not ensure eligibility or guarantee payment, so verify Medicaid eligibility first.
  • Drug-specific forms apply instead for atypical antipsychotics, growth hormone, hemophilia agents, Hemlibra, Synagis, Botox, hepatitis C DAAs and extended-release or second-level opioid requests.
  • The drug section asks whether the drug is physician-administered and needs NDC, strength, dosage form, dosage schedule, quantity and day supply.
  • Drug criteria are posted with the Alaska Medicaid Preferred Drug List on the medication prior authorization page. The current form revision is 10/03/2022.
