Alaska Medicaid Service Authorization Request (SA)

Alaska Medicaid providers use this form to request service authorization, the program's prior authorization, for medical services other than dental, long-term care and behavioral health. The requesting provider supplies facility and member details, the primary diagnosis code, dates of service, medical justification and the procedure codes, modifiers, units and charges requested, then signs an attestation of medical necessity.

Who completes it

The requesting provider completes items 1 through 20, and the signature must be that of the professional attesting that the content is accurate and meets Alaska Medicaid program requirements. Items 21 to 29 are reserved for the Alaska Medicaid Fiscal Agent.

Submitting and payment

How to submit it
Fax the request directly to the Alaska Medicaid Fiscal Agent, or send it by Direct Secure Messaging with the General Service Authorization coversheet.

Good to know

  • Item 12 asks whether the request is retroactive; if so, enter the date the services were actually rendered as the date of service.
  • Entering anything in the fiscal agent boxes, items 21 through 29, may cause the request to become invalid.
  • Authorization does not guarantee payment: it depends on the member's eligibility, so check that the member's identification card is current before rendering services.
  • Vision, private-duty nursing and physician-administered drugs have their own request forms, and Comagine Health handles utilization review for some services. Current revision 03/17/2023.
