Oregon OHP 3978: Pharmacy and Nutrition PA Request

OHP 3978 is the Oregon Health Authority's paper request for prior authorization of prescription drugs and oral nutritional supplements for fee-for-service (open card) Oregon Health Plan members. Prescribers send it, with the required documentation, to the Oregon Pharmacy Call Center when they cannot submit electronically. Reviewers apply the criteria in OHA's Oregon Medicaid PA Criteria guide.

Who completes it

The prescriber completes the request: provider name and NPI, client ID, treatment dates and diagnosis code, and the drug's name, strength and quantity. Section IV applies only to enteral, parenteral and IV products and oral supplements, and the Section V patient questionnaire only to supplements.

Submitting and payment

How to submit it
Fax the form with supporting documentation to the Oregon Pharmacy Call Center. Unlike medical prior authorization requests, it does not need the MSC 3970 EDMS coversheet.
Who pays
Drugs on OHA's Drug Carve-Out List are carved out of CCO benefits and reimbursed by OHA on a fee-for-service basis.

Good to know

  • The call center may ask for some or all of the form's information depending on the drug class requested, so review the criteria for that class before submitting.
  • FQHC, RHC, IHS and tribal 638 providers also enter their facility's pharmacy or clinic NPI and mark Other with their provider type instead of Pharmacy.
  • Most OHP members belong to a coordinated care organization (CCO), and CCO members use their CCO's pharmacy prior authorization form for drugs the CCO covers.
  • OHA updates the Drug Carve-Out List quarterly and notes that the list does not by itself determine coverage. Medical services and DME use the MSC 3971 instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who uses Oregon form OHP 3978?

Prescribers requesting fee-for-service Oregon Health Plan coverage of a prescription drug or oral nutritional supplement. Members enrolled in a coordinated care organization use their CCO's pharmacy form. OHA also pays some drugs fee-for-service because they are carved out of CCO benefits; its quarterly Drug Carve-Out List shows which.

Does OHP 3978 need an EDMS coversheet?

No. OHA's Prior Authorization Handbook says the pharmacy request does not require the MSC 3970 EDMS coversheet that medical and dental faxes need. It goes to the Oregon Pharmacy Call Center with the required documentation, and OHA describes it as the paper option for when an electronic submission is not possible.

Where are the criteria OHA uses to review OHP 3978?

OHA publishes its fee-for-service prior authorization criteria as the Oregon Medicaid PA Criteria guide, linked from the Pharmaceutical Services policy page alongside the form. Reviewing the criteria for the drug class before submitting helps the request include the clinical details the reviewer needs.
