Oregon WCD Form 3245: Return-to-Work Status

Oregon attending physicians use this one-page form to put a worker's release to work in writing. It offers three options: released to regular work, not released, or released to modified work with specific hours and limits on lifting, posture, hand and foot use. WCD calls this release the starting point for eligibility for the state's Employer-at-Injury and Preferred Worker return-to-work programs.

Who completes it

The attending physician or nurse practitioner completes the whole form, selects one work status option and signs it. The worker and employer do not complete any part.

Submitting and payment

How to submit it
Return it to the insurer or employer named in the 'Return form to' box; when work modifications are placed, changed or lifted, notify the insurer in writing within five days.
Who pays
WCD notes that providers may bill for responding to requests to clarify the specific limitations in a work release.

Good to know

  • Using Form 3245 is optional unless the insurer requests it, but every release to work must be written and specify restrictions, if any.
  • Do not release a worker to regular work and then restrict tasks; WCD's guidance calls that contradictory and asks for clear limits such as lifting up to 25 pounds floor to chest.
  • Modified work limits are set in pounds by frequency (one time, up to a third, a third to two thirds, over two thirds of the day) and in hours.
  • The form also asks whether the worker is expected to materially improve with treatment or time, and the next scheduled appointment date.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do I have to use Form 3245 for an Oregon work release?

Not always. WCD's handbook for physicians says the form is not required unless the insurer requests it. A release must still be in writing and specify any restrictions, and the insurer must be notified in writing within five days when work modifications are placed, changed or lifted.

Why does a clear Form 3245 matter for the worker?

WCD says the attending physician's work release is where eligibility for its return-to-work programs begins. A vague or inconsistent release can make the worker ineligible for Employer-at-Injury Program or Preferred Worker Program benefits, which pay for worksite modification, wage subsidies and employment purchases.
