Oregon WCD Form 2278T: Thoracic Range of Motion

Oregon examiners use this short WCD worksheet to report thoracic spine range of motion at a closing or impairment-focused exam under Bulletin 239. It covers thoracic flexion and right and left rotation, each calculated from paired inclinometer readings at T1 and T12. The form is returned with the examination report, and the insurer or the Division uses the recorded maximum angles to rate impairment.

Who completes it

The examining physician records the measurements, prints name and title, and signs with the exam date. Oregon attending physicians, including chiropractic physicians, and medical arbiters make impairment findings; naturopathic physicians do not.

Submitting and payment

How to submit it
Send it with the closing report to the insurer or self-insured employer, which Bulletin 239 expects within 14 days of the exam that found the worker medically stationary.
Who pays
The insurer pays the lesser of the billed amount or the OAR 436-009-0040 physician fee schedule amount, and the worker cannot be billed for care on an accepted claim.

Good to know

  • WCD norms printed on the form are 50 degrees thoracic flexion and 30 degrees rotation to each side.
  • Thoracic flexion is the T12 angle compared to T1 when flexed, minus the erect angle, which the form calls the angle of minimum kyphosis.
  • Rotation angles subtract the T12 reading from the T1 reading for each side, and each set must pass the 10 percent or 5 degree consistency check.
  • Measurements taken during acute spasm are recorded for comparison only, and the worker must be re-examined after the spasm resolves, per Bulletin 239.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What movements does Oregon Form 2278T record?

Three: thoracic flexion, thoracic right rotation and thoracic left rotation. For each, the examiner enters the paired readings, the calculated angle, whether the measurements fall within 10 percent or 5 degrees of each other, and the maximum angle obtained. Measurements are not averaged; the greatest valid angle is reported.

Do I need all three spinal forms for one exam?

No. Bulletin 239 asks for active range of motion in the affected spinal region, so the examiner completes the form for each region involved in the accepted condition: 2278C for cervical, 2278T for thoracic and 2278L for lumbar.
