Oregon DMV 735-24A Bioptic Limited Vision Report

Oregon's Limited Vision Condition Program lets people with best corrected acuity between 20/80 and 20/200 drive using a bioptic telescopic lens, and this report is the vision specialist's certification that the person qualifies. The specialist confirms prognosis, visual field, benefit from a bioptic and fitting, and states whether night driving is safe. A new certification is required every two years.

Who completes it

The applicant fills in identity and address details; a licensed vision specialist completes the certification checkboxes after an exam within the last six months, then signs the form and sends it in.

Submitting and payment

How to submit it
The vision specialist submits it directly to DMV Driver Specialty Services by fax or mail, as the applicant instructions on the form direct.

Good to know

  • Eligibility: better-eye acuity with best correction no better than 20/80 and expected to be no worse than 20/200 within 24 months, and a field of at least 120 degrees horizontally and 80 vertically.
  • Licensees start with a Class C license restricted to daylight and bioptic use. The daylight restriction lifts only when this report clears night driving and a training specialist certifies completed night training.
  • Before the drive test, the person must be fitted with a bioptic on the carrier lens, pass the knowledge test, and complete DMV-certified rehabilitation training on a special learner's permit.
  • Drivers under 18 must also meet provisional license requirements, including a regular instruction permit for at least six months and a driver's education course.

Frequently asked questions

Who sends Oregon's 735-24A to DMV?

The vision specialist. The applicant instructions say to take the certificate to a licensed vision specialist of their choice for an examination and ask the specialist to complete it and submit it to DMV Driver Specialty Services by fax or mail.

How recent must the exam be for form 735-24A?

The date of examination must be within the last six months. After licensing, the person must provide a new certification from a licensed vision specialist every two years to stay in Oregon's Limited Vision Condition Program.

Can a bioptic driver in Oregon drive at night?

Not at first. The license carries a daylight-only restriction, removed only when a vision specialist indicates on a Report of Limited Vision Examination that the person's vision does not prohibit safe night driving and a rehabilitation training specialist certifies completed nighttime training.
