South Carolina SCDMV 412-NC Vision Exam Certificate

South Carolina applicants for a non-commercial beginner's permit or driver's license who do not meet SCDMV's vision standard at the counter bring this certificate to a licensed eye care professional. The examiner records distant acuity for each eye and both eyes, then answers questions on corrective lenses, a weaker eye worse than 20/200, daylight-only driving, permanent impairment and earlier rechecks.

Who completes it

The patient completes the identity block and signs in the examiner's presence; a licensed eye care professional completes the acuity results and Sections A to C and signs the certification with license type and professional number.

Submitting and payment

How to submit it
The applicant brings it to an SCDMV branch; it stays valid for 36 months from the date of the vision examination.

Good to know

  • SC's non-commercial standard: 20/70 or better in at least one eye; if the weaker eye is worse than 20/200, the stronger eye must be 20/40 or better; or 20/70 with both eyes together.
  • The standard must be met without a telescopic lens or other attachment. Bioptic users need the separate 412-LV low vision certificate.
  • Confirm the individual is not applying for a CDL; commercial applicants need form 412-CDL, which applies the federal 20/40 and 70-degree standard.
  • Do not complete or sign if vision falls short, and do not return the form to someone needing new lenses until they are fitted. Alterations or erasures void it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

What is South Carolina's vision requirement for a regular license?

With or without corrective lenses: 20/70 or better in at least one eye, or, if the weaker eye is worse than 20/200, 20/40 or better in the stronger eye. A person worse than 20/70 in each eye can still qualify at 20/70 or better with both eyes together.

Can a bioptic driver use the 412-NC?

No. The 412-NC certifies vision met without a telescopic lens or other attachment. Drivers with low visual acuity who use bioptic telescopic lenses need form 412-LV under SC Code 56-1-222, which has its own standards and a 12-month validity.

Does the patient need to sign the 412-NC in front of the eye doctor?

Yes. The examiner certifies that the patient signed in their presence, along with certifying that the exam record is true and that every question was answered. The patient section also asks for email address, cell phone number and SC license number.
