South Carolina SCDMV 412-CDL Vision Exam Certificate

South Carolina CDL and commercial learner's permit applicants who need an outside eye exam bring this certificate to a licensed eye care professional. The examiner records distant acuity with and without correction and the horizontal field for each eye against the federal standard in 49 CFR 391.41(b)(10), then answers questions on corrective lenses, permanent sight impairment and rechecks.

Who completes it

The patient completes the identity block and signs in the examiner's presence; a licensed eye care professional completes the vision results and Sections A to C and signs the certification.

Submitting and payment

How to submit it
The applicant brings the completed certificate to an SCDMV branch; it is valid for 36 months from the date of the vision examination.

Good to know

  • The standard is 20/40 or better in each eye, with or without correction, and at least 70 degrees in the horizontal meridian in each eye, met without a telescopic lens or other attachment.
  • If the individual does not meet the standard, the examiner should not complete the rest of the form or sign the certification.
  • Do not hand the form back to someone who needs corrective lenses until the new lenses are fitted; acuity with the new prescription has its own line.
  • This certificate is separate from the federal DOT medical exam, and any alteration or erasure voids it. Non-commercial applicants use 412-NC.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

How long is South Carolina's 412-CDL valid?

36 months from the date of the vision examination. The form is void if there are any alterations or erasures on it, and the examiner certifies that every question was answered and that the patient signed in their presence.

What vision standard does the SC 412-CDL apply?

The federal minimum in 49 CFR 391.41(b)(10): distant acuity of 20/40 or better in each eye, with or without corrective lenses, and a field of vision of at least 70 degrees in the horizontal meridian in each eye. Telescopic lenses or other attachments cannot be used to meet it.

Does the 412-CDL replace the DOT medical card?

No. It is an SCDMV vision certificate for CDL and commercial learner's permit applicants, separate from the federal DOT medical examination. It records only distant acuity, horizontal fields, corrective lens need, permanent sight impairment and whether an earlier recheck is needed.
