DC DMV Mature Driver Fitness to Drive Certification

Every District of Columbia driver aged 70 or older must have a licensed medical practitioner certify their fitness to drive at license renewal, so this one-page form comes up routinely in primary care. The practitioner answers one question, whether the applicant can safely drive a vehicle, adds any comments and signs.

Who completes it

The driver completes Section A with customer information. A licensed medical practitioner must complete Section B: name, identification number, contact details, the yes-or-no safe driving finding, comments, signature and date.

Submitting and payment

How to submit it
Return the printable form with the renewal, or complete the certification through DC DMV's online transaction. DMV may issue a 45-day temporary license while it waits for the certification.
Who pays
The form lists no fee.
