TriMet Honored Citizen Form 230386: Provider Section

TriMet's Honored Citizen Application gives riders with a permanent or temporary disability a photo ID card for the reduced Honored Citizen fare in the Portland area. When the applicant has no Medicare card, Social Security verification or other qualifying proof, a health care provider completes the certification on the back. The applicant then submits the form in person, where TriMet takes the photo.

Who completes it

The applicant completes the front and signs a release naming the provider who may verify the disability. On the reverse, a physician, physician assistant, licensed clinical social worker, certified alcohol and drug counselor, QMHP or registered nurse practitioner records a license or certification number and certifies the disability.

Submitting and payment

How to submit it
The applicant brings the original, hand-completed application and a government-issued photo ID to the TriMet Customer Support Center in Portland within 30 days of the provider's completion date; TriMet does not accept a photocopy, fax or email.

Good to know

  • The provider checks one status: permanent disability (5-year Honored Citizen status), temporary disability of 12 months or less with its duration, or active enrollment in a drug or alcohol treatment program (up to 12 months).
  • For a permanent or temporary disability, the provider writes a specific description of the disability on the form or attaches one on official letterhead, and states whether an attendant is needed to ride.
  • Other routes skip the provider section: Social Security benefit verification, a Medicare card with photo ID, a certified agency's TriMet stamp, age 65 or older, or military or veteran documentation.
  • TriMet's LIFT paratransit application has no clinician section; LIFT eligibility is decided through an in-person interview and physical assessment at TriMet's Mobility Center.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can certify a disability for a TriMet Honored Citizen card?

The form lists physicians, physician assistants, licensed clinical social workers, certified alcohol and drug counselors (CADC), qualified mental health professionals (QMHP) and registered nurse practitioners. The provider certifies having examined the patient and that the disability substantially limits one or more major life functions, and TriMet may contact the provider to verify.

How long does the patient have to turn in the TriMet form after the provider signs?

The application must reach TriMet within 30 days of the date the health care provider completed the certification section. After that the provider section is out of date and the patient needs a new one, so the patient should plan the in-person visit for the photo soon after the appointment.

Can the TriMet Honored Citizen application be faxed or emailed?

No. TriMet will not accept a photocopy, fax or email of the form, and the application must be completed by hand. First-time and renewal applicants submit it in person with a government-issued photo ID because TriMet takes the photo for the card at that visit.
