Oregon DMV 735-265FPP: Family Disabled Parking Placard

Oregon DMV issues a family placard under ORS 811.609 when a family includes at least two people with disabilities, or when several households care for the same disabled person. A clinician meets this application when the family asks for the Certificate of Disability on page 2. DMV issues one placard for each vehicle listed as regularly transporting the disabled family members, valid for eight years.

Who completes it

An adult family member completes page 1 as the applicant and lists up to two additional drivers. The certifying clinician completes page 2: each disabled family member's name, date of birth, residence address and DMV customer number (or 'unknown'), a wheelchair-user checkbox for each, and a signature with license number.

Submitting and payment

How to submit it
The family sends the completed application to the Oregon DMV Driver Transactions Unit by fax, mail or email; incomplete applications are returned.

Good to know

  • Clinicians authorized to sign the certificate: doctors of medicine, osteopaths, optometrists, podiatrists, chiropractors, naturopaths, nurse practitioners and physician associates.
  • Disability follows ORS 801.387: severely limited mobility from paralysis or loss of limb use, vision loss beyond correction, or inability to walk 200 feet or to walk without an assistive device.
  • A replacement for a lost, stolen or destroyed family permit needs no new physician certification, and the replacement invalidates the permit issued before it.
  • If more than three family members qualify, the clinician lists the rest on an additional application. Certifying a statement known to be false is a crime under ORS 162.085.

Frequently asked questions

Who can sign the Oregon family disabled parking certificate?

Page 1 of form 735-265FPP lists the authorized signers: doctors of medicine, osteopaths, optometrists, podiatrists, chiropractors, naturopaths, nurse practitioners and physician associates. The signer certifies that every person listed on page 2 is disabled as defined by ORS 801.387 and records a license number, office address and phone.

How long is an Oregon family disabled parking placard valid?

ORS 811.609, reprinted on page 3 of the form, makes family placards valid for eight years from the date of issue, with renewal handled by DMV rule. The statute also bars DMV from issuing or renewing a family placard unless a licensed physician certifies that the family includes at least two persons with disabilities.

Who holds an Oregon family disabled parking permit?

The permit is registered to the applicant, who must be an adult family member, not to a vehicle or a driver. Placards are held by the applicant and the additional drivers named on page 1, and the permit cannot be transferred to another person or organization.
