Nebraska Application for Handicapped Parking Permit

Nebraska residents use this application for a permanent or temporary handicapped parking permit, including renewals, second permits and replacements. A licensed physician, physician assistant or nurse practitioner completes the Medical Certificate for a mobility, visual, respiratory or cardiac condition. The Department of Motor Vehicles mails the permit to the applicant once the application is processed.

Who completes it

The applicant completes the top portion and signs. A licensed physician, physician assistant or nurse practitioner completes the Medical Certificate in full and, for a temporary permit, marks three or six months.

Submitting and payment

How to submit it
Nebraska-licensed providers can submit new or renewal requests through the DMV's online handicap parking permit service. Otherwise the paper application is mailed to the DMV with a photocopy of acceptable proof of identity.
Who pays
The form does not mention a permit fee or address payment for the medical certificate.

Good to know

  • Temporary permits go to people whose limited mobility is expected to last no longer than one year. They are issued for three or six months and may be renewed one time only.
  • A permanent permit expires on the last day of the holder's birth month in the sixth year after issuance; renewals may be filed 180 days before expiration.
  • Nobody may hold more than two permits. Lost or stolen permits can be replaced twice; after that a new application with medical certification is required.
  • A provider not licensed in Nebraska cannot use the online service, so their patients file the paper form with the Medical Certificate completed.
