Nebraska Military Handicapped Plates Application

Veterans registered with the Nebraska Department of Veterans' Affairs Veterans Registry use this application to get military handicapped license plates, such as Veteran, Disabled American Veteran or Purple Heart designs. A licensed physician, physician assistant or nurse practitioner certifies a qualifying mobility condition. After the DMV approves it, the county treasurer issues the plates.

Who completes it

The veteran completes the Applicant and Vehicle Information sections and the plate message choices, then signs. A licensed physician, physician assistant or nurse practitioner completes the Medical Certification portion.

Submitting and payment

How to submit it
The application goes to the Nebraska DMV Driver and Vehicle Records Division in person or by mail, with a check or money order for the plate fee. An approval letter follows, and plates arrive at the county treasurer in four to five weeks.
Who pays
The veteran pays plate fees to the DMV and annual renewal fees to the county treasurer; the form does not address payment for the medical certification.

Good to know

  • The applicant and the registered vehicle owner must be on the Nebraska Veterans Registry; the Social Security number is requested only to verify registry status.
  • Medical criteria match Nebraska's other handicapped plates: impairments preventing unassisted travel of 200 feet, respiratory problems, AHA Class III/IV cardiac disease, or permanent loss of use of limbs.
  • The vehicle needs a Nebraska title in the applicant's name, with exceptions for leased vehicles and non-resident military.
  • The plates are not transferable and are for use only when the person with a disability enters or exits a vehicle parked in a designated space. The form is Revised 01/26.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
