Oregon 735-265: Disabled Person Parking Permit

Oregon residents use form 735-265 to apply for, renew or replace an individual disabled person parking permit placard or decal. Incomplete applications are not processed.

Who completes it

Submitting and payment

How to submit it
The applicant submits it by fax, email, mail to DMV Driver Transactions in Salem, or at any DMV office; DMV asks for 10 to 14 days for processing.
Who pays
The form does not address a permit fee or payment for the provider's certification.
