Oregon Vaccine Education Certificate (OHA 4683)

Oregon parents claiming a nonmedical (religious, philosophical or other) exemption from school or child care vaccines must first show they received vaccine education. This one-page certificate is the health care practitioner route: after reviewing vaccine benefits and risks with the parent, the practitioner marks the diseases discussed and signs. The parent then turns it in with the Certificate of Immunization Status.

Who completes it

An MD, DO, ND, NP or PA, or an RN working under the direction of an MD, DO, ND or NP, writes the parent's name, marks the diseases discussed, signs, checks their practitioner type and adds the clinic name. The parent adds the child's name and date of birth.

Submitting and payment

How to submit it
The parent turns in the signed certificate at the school or child care and fills out the nonmedical exemption section of the Certificate of Immunization Status there.

Good to know

  • Parents can skip the visit by finishing OHA's online Vaccine Education Module and printing the certificate at the end; it takes 15 to 60 minutes and progress is not saved.
  • Give one certificate per child when a parent asks for it for several children.
  • Check only the vaccines discussed: diphtheria/tetanus/pertussis, polio, varicella, MMR, hepatitis B, hepatitis A and Hib, which is required only for children younger than 5.
  • College students use a separate College Vaccine Education Certificate. Under ORS 433.267 the parent version may optionally record the reason: religious, philosophical or other.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Oregon Vaccine Education Certificate?

The certificate lets the signer check MD, DO, ND, NP or PA, or an RN working under the direction of an MD, DO, ND or NP. The practitioner reviews the benefits and risks of immunization with the parent under rules adopted under ORS 433.273, then signs and dates it.

Is there an alternative to a provider visit for an Oregon nonmedical exemption?

Yes. Parents can complete OHA's online Vaccine Education Module, offered in English, Spanish and Russian, and print the certificate at the end. They must save or print it before closing the module, because the certificate cannot be retrieved later.
