Virginia Report of Tuberculosis Screening (VDH)

Virginia's 'to whom it may concern' TB clearance documents the result of a TB evaluation for an employer, school, child care program or college. The evaluator works through a risk assessment and symptom screen, TST or IGRA results and any chest x-ray, then signs one of two conclusions: free of tuberculosis in a communicable form, or active TB cannot be ruled out and the person has been referred.

Who completes it

The health department, facility or clinician that performed the evaluation completes Sections I to V; the signer must be a clinician with prescriptive authority or a health department official.

Submitting and payment

How to submit it
Give the signed report to the individual, who provides it to the employer, school or program that requires TB screening.

Good to know

  • If symptoms consistent with TB are present, the form directs the clinician to call the local health department for immediate referral, even when the person prefers a private evaluation.
  • For a two-step TST, record both tests; for an IGRA, note whether T-Spot TB or QuantiFERON TB Gold Plus was used and the result.
  • A positive TST or IGRA sends the evaluator to Section IV, the chest x-ray date, location and interpretation, before a conclusion is signed.
  • VDH posts the report in Arabic, Simplified Chinese, Korean and Spanish as well as English, and it is a fillable PDF.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Virginia Report of Tuberculosis Screening?

The signature line is for a clinician with prescriptive authority or a health department official. The form also names the health department, facility or clinician that evaluated the person, then records the conclusion in Section V.

Does everyone need a TB test for the Virginia report?

No. If the TB risk assessment finds no symptoms and no risk factors, Section I states that a TST or IGRA is not indicated, and the evaluator moves to Section V. A person with a history of TB infection and no symptoms also needs no follow-up chest x-ray under Section I.
