South Carolina D-4024 Certificate of Immunization

South Carolina uses a single state form, the Certificate of Immunization, both to certify that a child has the vaccines required for child care and K-12 and to record a medical exemption. Providers generate it from SIMON, the state immunization registry, rather than downloading a blank. Parents give the printed certificate to the school or child care.

Who completes it

Only someone licensed to practice medicine, surgery or osteopathy, or that person's representative, may issue it as a certificate of medical exemption; health departments also issue certificates.

Submitting and payment

How to submit it
Print the certificate from SIMON and give it to the parent for the school or child care. DPH posts no blank PDF; its instructions are a DPH video playlist.

Good to know

  • There is no separate medical exemption form: DPH says the Certificate of Immunization meets both the certification and the medical exemption regulations.
  • Religious exemptions are signed by the parent and notarized, and that form is available only from the health department, in person or online.
  • A school principal may issue a Certificate of Special Exemption once, valid for 30 days, for transfer students awaiting records or students who could not get vaccinated in time.
  • The issuer is the South Carolina Department of Public Health, which took over immunization programs from DHEC when the agency split in July 2024.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where do I get a blank South Carolina Certificate of Immunization?

Who can issue a South Carolina medical exemption?

DPH says only an individual licensed to practice medicine, surgery or osteopathy, or that person's representative, may issue a certificate of medical exemption, and that it is done on the South Carolina Certificate of Immunization rather than a separate form.
