CAIR-ME: California School Vaccine Medical Exemption

Since January 1, 2021, every new medical exemption from California's school and child care immunization requirements must be issued through CAIR-ME, the California Immunization Registry medical exemption website. There is no paper form: a California-licensed MD or DO enters the permanent or temporary exemption online, and CAIR-ME generates the form the family gives the school.

Who completes it

The parent creates a CAIR-ME account, applies, and gives the physician the exemption application number. A California-licensed MD or DO registered in CAIR-ME issues the exemption, which must meet applicable American Academy of Pediatrics criteria, and prints the form.

Submitting and payment

How to submit it
Issue the exemption in CAIR-ME and print the form for the parent to give to the school or child care facility; schools register separately in CAIR-ME to view and confirm exemptions for their enrollees.

Good to know

  • An exemption can be issued no more than 12 months before a child first enters a grade span: birth to preschool, TK/K to 6th grade, or 7th to 12th grade.
  • CDPH must review exemptions when a school's immunization rate falls below 95 percent, a school fails to report rates, or a doctor writes 5 or more in a year, which is not a cap.
  • Physicians on probation or facing a pending accusation for immunization-related practices, or found by CDPH to be a public health risk, cannot issue new exemptions.
  • Exemptions end at the next grade span, at a temporary exemption's expiration date, or on revocation. Parents have 30 days to appeal a revocation to an independent expert panel.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can California physicians still write a paper medical exemption?

No. Since January 1, 2021, all new school and child care medical exemptions must be issued through CAIR-ME, under SB 276 and SB 714. Exemptions issued before 2021 for children already enrolled stay on file at the school and do not need to be uploaded into CAIR-ME.

How does a parent start a CAIR-ME medical exemption request?

The parent creates a CAIR-ME account, applies for an exemption and receives an application number, then gives that number to the child's physician. The physician registers in CAIR-ME, logs in to issue the exemption, and prints the form for the parents to give the school. CDPH's Parent and Physician CAIR-ME Guide is posted in 14 languages.

Does a child need a new CAIR-ME exemption after changing schools?

Not within the same grade span. The parent logs in to CAIR-ME, updates the school or child care facility information, and shows the new school a printed copy of the change. The new school can also log in to confirm the exemption is listed for it. A new grade span needs a new exemption.
