Iowa Certificate of Immunization (HHS) for School

Iowa children enrolling in licensed child care or school need a Certificate of Immunization showing age-appropriate immunizations that meet state requirements. Page 1 records dates and sources for DTaP, hepatitis B, varicella, PCV, polio, MenACWY, MMR and Hib; page 2 lists the July 2026 dose requirements by age and grade.

Who completes it

A physician (MD or DO), physician associate, nurse or certified medical assistant records the vaccine dates and signs the certification at the bottom of page 1.

Submitting and payment

How to submit it
Give it to the parent for the school or licensed child care. Certificates can also be printed from IRIS, Iowa's immunization registry.

Good to know

  • For a child with a history of natural chickenpox, write 'Immune to Varicella' in the varicella row instead of dates.
  • A local Board of Health or Iowa HHS representative may review the certificate for audit purposes.
  • HHS-approved alternative certificates, such as EHR printouts, are accepted when covered by a Statement of Approval for Alternative Certificate of Immunization.
  • OPV doses given on or after April 1, 2016 do not count toward the polio requirement.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Iowa Certificate of Immunization?

A physician (MD or DO), physician associate, nurse or certified medical assistant prints their name, signs and dates the certification that the child has age-appropriate immunizations meeting the requirement for licensed child care or school enrollment. A certificate printed from IRIS is also accepted.

Can an Iowa clinic use its EHR printout instead?

Only with HHS approval. Iowa accepts HHS-approved alternative certificates under a Statement of Approval for Alternative Certificate of Immunization, in addition to the state form and IRIS-generated certificates. Without that approval, the state form or an IRIS printout is the route.
