Iowa Medical Certificate of Immunization Exemption

Iowa HHS's medical exemption certifies that required vaccines would be injurious to the health of a child or a household member, or that giving MMR or varicella now would violate the 28-day spacing after a previous live vaccine. The clinician checks only the vaccines that are medically contraindicated and can set an expiration date for review.

Who completes it

An Iowa-licensed physician (MD or DO), nurse practitioner or physician associate selects the reason and vaccines, records an expiration date if needed, and signs with their Iowa license number.

Submitting and payment

How to submit it
The applicant, or the parent or guardian of a minor, submits it to the admitting official of the school or licensed child care center.

Good to know

  • The household-member contraindication applies only to MMR and varicella; other vaccines must be contraindicated for the child.
  • For the live vaccine spacing reason, the exemption applies only to children without prior doses of that vaccine and expires in no more than 60 days.
  • A child with a medical exemption may be excluded from child care or school during an outbreak, for several days to over a month.
  • The current version is dated July 2026. Religious exemptions use a separate parent form, which no longer needs a notary since July 1, 2024.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign an Iowa medical immunization exemption?

Only an Iowa-licensed physician (MD or DO), nurse practitioner or physician associate. The form says it is valid only with such a signature, and the signer enters their Iowa license number. Nurses and certified medical assistants, who can sign the regular Certificate of Immunization, cannot sign the exemption.

Does an Iowa medical exemption expire?

Only if the clinician sets a date. When the exemption should end or be reviewed later, an expiration date is recorded on the certificate. Exemptions based on live vaccine spacing must carry an expiration date of no more than 60 days.
