Iowa Provisional Certificate of Immunization (60 Days)

Iowa allows provisional enrollment in school or licensed child care for a child who has received at least one dose of each required vaccine but has not finished the series, or for a transfer student moving between US elementary or secondary schools. This certificate records the immunizations given, the vaccines still required and an expiration date.

Who completes it

A physician (MD or DO), physician associate, nurse or certified medical assistant completes the record, remaining vaccines and expiration date, and signs to certify they informed the family of the provisional enrollment requirements.

Submitting and payment

How to submit it
The applicant submits it to the admitting official of the school or child care center, and a copy goes to the applicant, parent or guardian.

Good to know

  • Provisional enrollment must be as rapid as medically feasible and cannot exceed 60 calendar days, starting on the date the certificate is signed.
  • The certificate is valid only when completed in full, including the expiration date and the list of remaining vaccines.
  • A transfer student is one moving from one US domestic elementary or secondary school to another.
  • The remaining vaccines listed are those the child needs to qualify for the standard Iowa Certificate of Immunization.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
