Louisiana Certificate of Immunizations from LINKS

Louisiana schools must receive a certificate of required immunizations before enrollment under R.S. 17:170. Providers with access to LINKS, the Louisiana immunization registry, print the Certificate of Immunizations with the student's immunization history prepopulated, which is why there is no blank form to download.

Who completes it

An immunization provider with LINKS access generates and prints the certificate. LINKS fills in the vaccine history, and the provider can print it only when the certificate is up to date and not expired.

Submitting and payment

How to submit it
Print it from LINKS and give it to the parent, who gives it to the school or child care facility.

Good to know

  • LDH's school immunization guide says providers can print the certificate only if it is up to date and not expired.
  • The certificate pulls the vaccine history straight from LINKS rather than from a form the provider fills in by hand, so registry entries drive what prints.
  • Exemptions are documented separately on the Louisiana Certificate of Exemption, not on the LINKS certificate.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is there a blank Louisiana Certificate of Immunizations?

No. The certificate is generated from LINKS with the student's immunization history prepopulated. Providers with LINKS access print it when the child is up to date and the certificate has not expired, and the family gives it to the school.

Who can print a Louisiana Certificate of Immunizations?

Immunization providers with LINKS access. The LDH school immunization guide states that they can print the Certificate of Immunizations if it is up to date and not expired, with the history prepopulated by LINKS. Schools need the certificate before enrollment under R.S. 17:170.

What if a Louisiana student has a vaccine exemption?

Exemptions are not part of the LINKS Certificate of Immunizations, which prints only when a child is up to date and the certificate has not expired. LDH uses a separate form, the Louisiana Certificate of Exemption, to document an exemption for school or child care.
