Alabama Certificate of Immunization (COI) from ImmPRINT

Alabama law (Code of Alabama 16-30-4) requires a Certificate of Immunization on file for every child attending school or licensed child care. The only approved COI is printed from ImmPRINT, the state immunization registry, and shows the school-required doses that are valid along with a date of expiration tied to the next required immunization.

Who completes it

A licensed physician's office or ADPH county health department enrolled in ImmPRINT prints the certificate, which carries the issuing site's name, address and date of issue. Pharmacies and other sites without a medical authority may print COIs only for children who are up to date.

Submitting and payment

How to submit it
Print it from ImmPRINT or ImmPRINT Webservice on plain paper and give it to the parent, who gives it to the school or child care facility.
Who pays
ADPH provides ImmPRINT and its required training to enrolled provider sites; the certificate itself carries no fee or billing code.

Good to know

  • Since June 1, 2016, COIs created by EHR vendors are not accepted; ADPH restated this in a letter dated May 7, 2019.
  • The COI shows only valid school-required shots; doses ImmPRINT considers invalid do not print on it.
  • ImmPRINT sites must document administered shots in the registry; view-only provider sites are not supported, and ADPH training is required for every site.
  • Medical exemptions use a separate Certificate of Medical Exemption from ImmPRINT, and religious exemptions are issued only by county health departments.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I print an Alabama Certificate of Immunization from my EHR?

No. Since June 1, 2016, the only approved COI is printed from ImmPRINT or ImmPRINT Webservice on plain paper, and ADPH's May 7, 2019 letter confirmed that EHR vendor-created certificates are not accepted. The printed certificate states that a licensed physician or qualified ADPH employee is responsible for its content.

What is the expiration date on an Alabama COI?

Each certificate carries a date of expiration labeled as the next required immunization. It tells the school or child care facility when the child's next school-required dose is due, so the certificate on file reflects whether the child remains current under Alabama's requirements.
