California TB Risk Assessment for School Staff (CDPH)

California law requires pre-K, K-12 and community college employees, contractors, and volunteers with frequent contact with pupils to complete a TB risk assessment, with testing if risk factors are found. This CDPH and California TB Controllers Association packet, dated 2/27/2026, holds the required questionnaire, background guidance, the Certificate of Completion the employer keeps, and FAQs.

Who completes it

A health care provider, meaning any person or organization licensed, certified or otherwise authorized by state law to furnish health services, administers the questionnaire and signs the Certificate of Completion. CDPH discourages letting employees or volunteers assess themselves.

Submitting and payment

How to submit it
Give the signed Certificate of Completion to the employee or volunteer, who submits it to the school district, private school, child care center or community college.

Good to know

  • Testing is recommended if any of three boxes is checked: TB signs or symptoms; at least 1 month of birth, travel or residence in a country with an elevated TB rate; or close TB contact.
  • With a documented positive TB test or past TB disease, a symptom review and chest x-ray at hire (if none in 6 months) replaces future risk assessments when the x-ray shows no TB.
  • Risk assessments repeat every four years unless otherwise required, under Education Code 49406 and 87408.6 and Health and Safety Code 1597.055 and 121525 to 121555.
  • A K-12 employee moving to another school or district needs no new assessment if they show a certificate within 60 days of hire or the prior school verifies one on file.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How often do California school employees need a TB risk assessment?

At hire and then every four years, unless otherwise required. The provider repeats the questionnaire to look for new risk factors and orders testing based on the answers. Someone with a documented positive test followed by a chest x-ray free of infectious TB no longer needs the assessment.

Who can sign the California school staff TB Certificate of Completion?

A health care provider, which the form defines as any organization, facility, institution or person licensed, certified or otherwise authorized by state law to deliver health services. The signer certifies the person has no risk factors or, if risk factors were found, was examined and found free of infectious TB.

Can a school employee get a TB test instead of the risk assessment?

Yes. The CDPH FAQs say a TB test, and an examination if necessary, may be completed instead of the TB risk assessment. New employees with a recent negative TB test should check with their employer about what is needed at the time of hire.
