Virginia TB Risk Assessment Tool (Age 6 and Older)

Virginia's one-page TB screening and risk assessment, with a two-page user guide, helps decide whether an asymptomatic person 6 or older should be tested for latent TB infection. It starts with a symptom screen, then a checklist of risk factors; any checked risk means testing is recommended. Its findings support the Report of Tuberculosis Screening that employers and schools receive.

Who completes it

A licensed MD, PA, NP, RN or LPN may complete the symptom screen and risk assessment, recording name, title and signature; an RN or LPN doing further evaluation needs an order from a clinician with prescriptive authority.

Submitting and payment

How to submit it
Keep the completed assessment in the patient's record; its result feeds Section I or III of the Virginia Report of Tuberculosis Screening when a clearance is needed.

Good to know

  • Risk factors listed: birth, travel or residence of 3 months or more in a country with an elevated TB rate, conditions raising progression risk, current or planned immunosuppression, and close contact with infectious TB.
  • Re-testing is only for people who previously tested negative and have new risk factors since the last assessment.
  • The tool does not replace testing mandated by statute, regulation or policy, such as for health care workers or staff of correctional facilities and shelters.
  • A separate VDH tool covers children under 6. Both are posted in Arabic, Simplified Chinese, Korean and Spanish, and this edition is dated March 2023.
