Confidential Oregon Morbidity Report (OHA)

Oregon clinicians are required by law to report confirmed or suspected diagnoses of specified diseases and conditions under OAR chapter 333, division 18, and this one-page fillable form is the Oregon Health Authority's fax option for doing so. It goes to the patient's local health department. OHA accepts any equivalent document that contains all the required information.

Who completes it

The reporting clinician completes the whole page: patient details and language, the disease or condition with onset and lab information, and the reporting physician's contact details, including an alternate contact such as a nurse. The patient does not complete any part.

Submitting and payment

How to submit it
Fax it to the local health department for the patient's county of residence, one sheet per patient, with no cover page needed; out-of-state residents go directly to OHA in Portland. OHA also offers a secure Online Morbidity Report portal that routes reports by the patient's address, and electronic case reporting.

Good to know

  • The form asks whether the patient has already been told the diagnosis and whether health department staff should call the clinician before trying to interview the patient.
  • Diagnosis is marked as confirmed by lab results, supported by lab results, or clinical or suspect only, with the specimen date and lab name when testing was done.
  • The patient's language is captured with checkboxes for English, Spanish, Russian or other, alongside a parent or guardian field for minors.
  • Oregon's reporting rules set civil penalties for failing to report, and each day out of compliance counts as a new violation.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where do I send an Oregon morbidity report?

Send it to the local health department where the patient lives. OHA posts the confidential fax numbers for each county on its disease reporting page. If the patient lives outside Oregon, fax the report directly to the Oregon Health Authority in Portland instead of a county office.

Do I have to use the Oregon Morbidity Report form?

No. OHA says clinicians can call to discuss a report or fax the required information on this form or any equivalent that includes everything required. Providers can also enter reports in the secure Online Morbidity Report portal or set up electronic case reporting from the EHR.

Can I use OHA's disease-specific case report forms instead?

Generally no. OHA's index of disease-specific case report forms states they are for use by local health departments. Clinicians report on the morbidity report, an equivalent, the online portal or eCR, and the local health department handles any disease-specific investigation form.
