Oregon DWDA Attending Physician Follow-up Form

After a patient who received a Death with Dignity Act prescription dies, from the medication or any other cause, the prescribing physician reports the circumstances on this five-page form, Rev. 4/26. The Oregon Health Authority matches prescription recipients against death certificates and uses the form to learn whether death followed ingestion, any complications, end-of-life care and the concerns behind the patient's request.

Who completes it

The attending (prescribing) physician signs page 1 and completes Part B on the patient's status and reasons. Part A, on the ingestion and death, is completed by whoever was present: the attending physician, another licensed provider or a volunteer.

Submitting and payment

How to submit it
Send it to OHA by secure email, fax or regular mail within 10 calendar days of the patient's ingestion of the medication or death from any other cause, whichever comes first.

Good to know

  • OHA accepts the form only if the attending physician signs it, whether or not they were present at the death.
  • If the patient died of the underlying illness or another cause, such as terminal sedation, sign page 1 and submit page 1 only.
  • If the cause of death is unknown, the form asks the physician to contact the family or the patient's representative.
  • Part A asks for ingestion and death dates, time to unconsciousness and death, location, complications such as vomiting or seizures, EMS activation and hospice status.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is the Oregon Death with Dignity follow-up form due?

Within 10 calendar days of the patient's ingestion of the lethal medication or death from any other cause, whichever comes first. It goes to OHA by secure email, fax or mail, and OHA keeps the information strictly confidential.

What if the Oregon DWDA patient died of their illness, not the medication?

Check underlying illness on page 1, sign, and stop; submit page 1 only. Parts A and B are completed only when the patient died from ingesting the lethal dose. OHA's guidance for death certificates is to list the underlying terminal disease as the cause and mark the manner of death natural.
