Oregon DWDA Psychiatric Consultant Compliance Form

Under Oregon's Death with Dignity Act, when the attending or consulting physician refers a patient for evaluation, a psychiatrist or psychologist decides whether a psychiatric or psychological disorder, or depression, is impairing the patient's judgment. This one-page form, Rev. 5/18, records the evaluation and the consultant's conclusion. It becomes part of the attending physician's compliance packet sent to the state.

Who completes it

The psychiatrist or licensed psychologist who did the evaluation completes the form, listing the medical diagnosis, the dates of examination and a summary of the evaluation, and signs with their title, such as M.D. or Ph.D.

Submitting and payment

How to submit it
Deliver it to the attending or prescribing physician, who sends it to the OHA Center for Health Statistics within 7 calendar days of writing the prescription.

Good to know

  • The form is needed only when the attending or consulting physician refers the patient; otherwise those physicians record the mental status finding on their own forms.
  • The signed statement is that the patient is not suffering from a psychiatric or psychological disorder, or depression, causing impaired judgment, in conformance with ORS 127.825.
  • The attending physician's form asks for the consultant's name and date and requires this form to be attached to their filing.
