Oregon ODE Medical Statement for Special Meals

Oregon child nutrition sponsors, including schools, child care centers and summer meal sites, use this statement when a participant needs a meal accommodation because of a disability. The signer describes the major life activity or bodily function the impairment affects, lists foods to omit or avoid, and gives substitutions. The sponsor then records the accommodations it will make and signs Part III.

Who completes it

A state-licensed health care professional authorized to prescribe, or a registered dietitian for school meals and CACFP, completes and signs Part II. The parent, guardian, adult participant or organization completes Part I, and the sponsoring organization completes Part III.

Submitting and payment

How to submit it
Return the form to the person named in the Submit this form to field, which the sponsoring organization fills in with the staff member collecting completed forms.

Good to know

  • Oregon's list of state-licensed signers includes MDs, DOs, naturopathic doctors, PAs, certified nurse practitioners, clinical nurse specialists, dentists (DMD or DDS) and optometrists.
  • Non-medical requests and milk substitutions use the separate Meal Preference Request Form, not this medical statement.
  • The statement is posted in English, Spanish, Russian, Simplified and Traditional Chinese, Vietnamese, Arabic, Chuukese and Somali.
  • The form counts as incomplete until the sponsoring organization fills in Part III with the accommodations made and signs it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a registered dietitian sign the Oregon medical statement?

Yes, for school meals and the Child and Adult Care Food Program. The form cites 7 CFR 210.10(m) and 7 CFR 226.20(g)(1), which authorize registered dietitians to complete and sign medical statements for those programs. Otherwise Part II needs a state-licensed health care professional authorized to write prescriptions.

Does the Oregon medical statement cover child care and summer meals?

Yes. Oregon uses one statement across school meals, the Child and Adult Care Food Program and the Summer Food Service Program, and its instructions give examples such as a school or a child care center. The sponsoring organization enters its name and the site where meals are served.

What should the clinician write on the Oregon medical statement?

Part II asks three things: the major life activity or bodily function affected by the physical or mental impairment that restricts the diet, the foods to omit or avoid, and the foods to substitute with recommended alternatives. Then print your name, sign and date it.
