Paid family leave and state disability certifications for providers

Medical certifications for state disability insurance and paid family and medical leave programs, from California SDI to Minnesota and Delaware paid leave, plus private-plan versions.

Several states run disability insurance or paid family and medical leave programs, and each asks the clinician to certify the condition. California's EDD uses the DE 2501 for State Disability Insurance and the DE 2501F for Paid Family Leave. New York uses the DB-450 for disability benefits and the PFL-4 for family care, and New Jersey uses the DS-1 and FL-1. Washington, Massachusetts, Oregon, Connecticut, Colorado, Minnesota, Delaware and DC have paid leave certifications of their own.

Formats vary. Most California certifications go through SDI Online, Delaware's is online only, and Colorado providers can certify in My FAMLI+. Washington and Connecticut say providers may not charge for their certifications, and New Jersey's DS-1 says the same. Several programs, including Washington, Minnesota and Connecticut, accept a completed FMLA form instead. Employers with a private plan use their carrier's form, such as Unum's CL-1263-MA. 18 forms here have a Spanish version. Vero Documents fills PDF certifications from the visit note and the chart; you review and sign.

Frequently asked questions

How do I certify a California SDI claim on the DE 2501?

Most certifications are done in SDI Online, which needs a California license and a myEDD account. On paper, Part B, the Physician or Practitioner's Certificate, is mailed with the claimant's Part A within 49 days of the disability start; the PDF EDD posts is a sample only. Rubber-stamped signatures are not accepted. To extend a claim, EDD issues the DE 2525XX per claim, and it is returned within 20 days.

When is the New York DB-450 due?

The provider completes Part B, the Health Care Provider's Statement, and returns the form to the claimant within 7 days. The claimant files it with the employer's disability benefits carrier within 30 days, or with the Workers' Compensation Board if the employer is uninsured. Carriers often use their own claim form but must accept the DB-450. It is available in Spanish and 11 other languages.

Can I use an FMLA form for Washington Paid Family and Medical Leave?

Yes. Washington's Employment Security Department accepts an FMLA form such as the WH-380, or a provider note, if it contains the information state rules require. ESD's own certifications, such as the Medical Certification and the Certification of Birth, are bilingual English and Spanish PDFs. Return the form to the patient within 7 calendar days; the patient uploads it to their Paid Leave account. ESD says providers cannot charge a fee for it.

Do I fill out the state's form or the insurer's form?

It depends on how the employer covers paid leave. Employers with an approved private or equivalent plan use their carrier's form, such as Unum's CL-1263-MA in Massachusetts or CL-1329-OR in Oregon, or Guardian's MA PML-1. State-plan claims use the state's own form, like the Massachusetts DFML Certification of Your Serious Health Condition, which you may send directly to DFML or return to the patient to upload.
