DC PFL-PMC: Prenatal Leave Medical Certification

DC Paid Family Leave includes up to two weeks of prenatal leave for pregnancy-related medical care, usable in one-day increments, and this form is how the pregnant worker's health care provider certifies it. Prenatal leave covers check-ups, pregnancy-related treatment and diagnostic visits, and doctor-ordered bedrest. The claimant uploads the completed certification when filing for prenatal leave.

Who completes it

The pregnant claimant's licensed health care provider, such as an OB/GYN, midwife, family physician or nurse practitioner, completes and signs the provider part of the form.

Submitting and payment

How to submit it
The claimant uploads the completed form with the prenatal leave application in DC's online Paid Family Leave benefits portal.

Good to know

  • After the first claim, later prenatal visits can be documented with visit slips, visit summaries, a letter from the provider's office listing care dates, or an updated certification.
  • Prenatal leave is available throughout the pregnancy and is separate from the 12 weeks of parental leave after birth; a partner cannot claim prenatal leave for the pregnant worker's appointments.
  • The form is posted in English, Spanish, Amharic, Chinese, French, Korean and Vietnamese; its download page was last updated March 29, 2024.
