DC PFL-FMC: Family Leave Medical Certification Form

Workers in the District of Columbia who take Paid Family Leave to care for a family member with a serious health condition need this four-page certification. The claimant describes the care or companionship they will give; the family member's health care provider then documents the condition with ICD-10 codes, the leave needed and whether that care is reasonable and necessary. The claim cannot be filed until it is signed.

Who completes it

The claimant completes Part 1 first; the family member's treating licensed health care provider completes all of Part 2, Sections A through E, and signs.

Submitting and payment

How to submit it
The claimant uploads the completed form while filing the claim in DC's online Paid Family Leave benefits portal.

Good to know

  • Every Part 2 section must be completed or the form comes back to the provider for more information, and the claim may be denied.
  • Section B choices: pregnancy, overnight inpatient care, incapacity plus treatment, chronic condition, permanent incapacity, restorative surgery, preventative treatment, stillbirth, or none of the above.
  • Section D asks for your opinion on whether the patient requires care or companionship from the claimant and whether the care described in Part 1 is reasonable and necessary.
  • Translations are posted in Spanish, Amharic, Chinese, French, Korean and Vietnamese. A claimant's own illness uses PFL-MMC instead.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes the DC Family Leave Medical Certification Form?

The claimant fills in Part 1, including the family member's name, relationship and the care or companionship they will provide. The doctor or licensed health care provider treating the family member completes Part 2: provider details with license number, the qualifying condition and ICD-10 code, amount of leave, need for care and the signed certification.

When does a DC family leave claim need the PFL-FMC?

At the time of application. DC's Office of Paid Family Leave says claimants must include the medical certification when they apply, and the PFL-FMC instructions say family leave filing can be completed only after the family member's provider has completed and signed the form, so prompt completion matters.
