Guardian CO PML-2: Colorado Medical Leave Certification

Employers that cover Colorado FAMLI through a Guardian private plan use this claim packet when an employee takes paid medical leave for their own serious health condition, including pregnancy. The employee's health care provider completes the Certification for Employee's Own Serious Health Condition (CO PML-2): which condition category applies, the dates, and whether leave is continuous, reduced schedule or intermittent.

Who completes it

The employee's health care provider completes Parts A and B of CO PML-2 and signs with license number, state and specialty. The employee completes the request and the certification header, and the employer completes the employment verification.

Submitting and payment

How to submit it
The completed packet goes to Guardian's Paid Leave Claims Department by mail, fax, email or Guardian's secure upload channel, or the employee can file the claim online with Guardian.

Good to know

  • Part A categories: inpatient care, incapacity of more than three consecutive full calendar days plus treatment, pregnancy, chronic conditions, permanent or long-term conditions, and conditions needing multiple treatments.
  • For pregnancy, give the expected delivery date, the type of delivery (vaginal, cesarean or multiple births) and whether it involves a complication.
  • Intermittent leave asks for the frequency and duration of episodes over the next 6 months; reduced schedule asks for the patient's work capacity in hours per week.
  • If the infant is admitted to the NICU, Guardian notes the parents may qualify for additional neonatal care leave and should contact Guardian.

Frequently asked questions

Who completes the Guardian Colorado PML certification?

The employee's health care provider completes Part A (medical information) and Part B (amount of leave needed) of CO PML-2, limiting answers to the condition behind the leave, and signs under penalties of perjury with license number, state and medical specialty.

Does the Guardian Colorado PML packet cover pregnancy?

Yes. The employee can select pregnancy as the leave reason, and the provider checks the pregnancy category in Part A with the expected delivery date, delivery type and whether there is a complication. Part B then describes the continuous, reduced schedule or intermittent leave needed.
