CT Paid Leave Caregiver Certification (Family Member)

Connecticut workers taking CT Paid Leave to care for a relative need this five-page certification (CTPL-0009) from the relative's health care provider. Part A shows how the patient's condition qualifies as serious, and Part B states what care the patient needs and how much leave the caregiver requires. The form notes that claims are administered by Continental American Insurance Company.

The applicant completes their own details, the relationship, and the family member's and provider's information. The family member's health care provider completes Parts A and B and signs with credentials under penalties of perjury.

Good to know

  • Covered relationships include spouse, child, parent, sibling, grandparent, grandchild, in-laws and step-relatives, plus anyone related by blood or affinity whose relationship is equivalent to one listed.
  • Part B asks when the condition started, how long it will last, and whether the patient needs psychological comfort, help with basic needs or transportation, or someone at appointments.
  • Give actual start and end dates for continuous, reduced-schedule or intermittent leave; 'To be determined' or 'Unknown' results in the application being denied.
  • Fully completed FMLA medical documentation, such as WH-380-F, is accepted instead, and the applicant uploads a separate Statement of Family Relationship.
