CT Paid Leave Military Caregiver Certification

Connecticut workers can use CT Paid Leave to care for a spouse, parent, child or next of kin who is a current service member with a serious injury or illness. The service member's health care provider completes this three-page certification (CTPL-0010), covering line-of-duty status, the care needed, the DoD casualty classification and the amount of leave.

Who completes it

The applicant completes their own, the relationship and the service member's details. The service member's provider, whether DoD, VA, TRICARE network or non-network, or another provider under 29 CFR 825.125, completes the medical information and signs with credentials.

Submitting and payment

How to submit it
The applicant uploads it to the CT Paid Leave Aflac portal as a Caregiver Certification Form; providers may also send completed certifications directly to the claims administrator.

Good to know

  • Question 5 asks whether the injury or illness was incurred in the line of duty on active duty, or existed before active duty and was aggravated by it.
  • Classifications are Very Seriously Ill/Injured and Seriously Ill/Injured (internal DoD designations), Other Ill/Injured if it may make the member medically unfit for duty, or none of the above.
  • The form warns that estimates such as 'lifetime,' 'unknown' or 'indeterminate' may not be enough to determine coverage, so give dates, frequency and duration.
  • CT Paid Leave accepts a completed federal WH-385, or an Invitational Travel Authorization or Order, instead. Benefits are limited to 12 weeks in a 12-month period.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Does CT Paid Leave accept the federal WH-385 for military caregiver leave?

Yes. The application document checklist accepts fully completed FMLA medical documentation on Form WH-385 in place of the CT certification, uploaded as a Caregiver Certification Form. A copy of an Invitational Travel Authorization or Invitational Travel Order is also accepted, uploaded as Military Orders.

How many weeks of CT Paid Leave can a military caregiver receive?

CT Paid Leave benefits are limited to 12 weeks in a 12-month period, even though federal and Connecticut FMLA provide up to 26 weeks of job-protected leave for military caregivers. The leave is for a spouse, parent, child or next of kin of the service member.

What if the service member's condition fits none of the classifications?

The form tells the employee they may still qualify for leave to care for a family member with a serious health condition under the FMLA definition in 29 CFR 825.113. In that case they may be asked to complete the federal WH-380-F or an employer form seeking the same information.
