New Jersey FL-1: Family Leave Insurance Claim

New Jersey Family Leave Insurance claims use the FL-1. When an employee takes leave to care for a seriously ill family member, Part C Section 1 is the care recipient's medical certificate: diagnosis with ICD code, whether full-time care is needed, and when care started and should end. Bonding claims skip Part C entirely.

Who completes it

The claimant completes Parts A and B, and Part D for a partial schedule; the care recipient's licensed healthcare provider completes Part C Section 1, and the care recipient signs Section 2 authorizing disclosure.

Submitting and payment

How to submit it
The claim is faxed or mailed to the Division within 30 days of the first day of leave; when the claimant files online, the provider submits the FLI medical statement (M-01) on the Division's secure site using the claimant's Form ID.
Who pays
The FL-1 prints no fee rule and names no payer for the provider's certification.

Good to know

  • If care is not full time, Section 1 asks how many days per week the patient needs care, which shapes the claimant's partial leave schedule in Part D.
  • Section 2 is the care recipient's authorization for their providers to disclose current health information to the claimant and the Division; a representative may sign under parental right, power of attorney or court order.
  • Claimants who do not claim all 12 weeks at once later receive form FL-3 to certify additional periods of leave.
  • Leave tied to domestic or sexual violence, where medical documentation does not apply, is supported with case documentation instead of a provider certificate.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the provider complete the FL-1 for bonding leave?

No. Part C is completed only when the claimant is caring for an ill family member. Bonding claims for a new child do not need the care recipient's medical certificate, so a provider is involved only in caregiving claims under New Jersey Family Leave Insurance.

Who signs Part C Section 2 of the FL-1?

The care recipient. Section 2 authorizes the care recipient's physicians to disclose current health information to the claimant and the Division of Family Leave Insurance. If the care recipient cannot sign, an authorized representative signs under parental right, power of attorney or court order.

Can the FL-1 medical certificate be completed online?

Yes, when the claimant filed online. The care recipient's provider uses the claimant's Form ID to complete the FLI medical statement (M-01) on the Division's secure website and receives immediate confirmation. Paper Part C is used only with a paper FL-1.
