New Jersey DS-1: Temporary Disability Claim (Part C)

New Jersey workers claim state Temporary Disability Insurance for their own illness, injury or pregnancy with the DS-1. Part C, the Medical Certificate on page 3, is where the treating provider certifies the disability period, diagnosis with ICD code and estimated recovery date. The Division of Temporary Disability and Family Leave Insurance uses it to decide benefits.

Who completes it

The claimant completes Part A (personal and disability information) and Part B (all employers in the past six months); the patient's licensed healthcare provider completes and signs Part C.

Submitting and payment

How to submit it
Paper claims are faxed or mailed to the Division, not both; if the claimant filed online, the provider enters the medical statement (M-01) on the Division's secure site with the claimant's Form ID instead. The claim is due within 30 days of the first day of disability.
Who pays
No fee may be charged: Part C states that N.J.S.A. 12:18-1.6 prohibits charging a fee to complete the form.

Good to know

  • For pregnancy, Part C asks for the estimated delivery date, pre-term and postpartum complications, the actual delivery date and the type of delivery.
  • Claims received more than 30 days after the disability began need a written reason, and benefits may be reduced or denied for late filing.
  • After approval the Division may send form P-30 to continue the claim; the claimant and provider can complete their parts online to keep benefits uninterrupted.
  • Employees whose employer has a private disability plan file with that carrier on its own form rather than the state DS-1.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a New Jersey provider charge for completing the DS-1?

No. Part C of the DS-1 states that N.J.S.A. 12:18-1.6 prohibits charging a fee to complete the form. The rule is printed directly above the medical certificate that the provider fills in for a state Temporary Disability Insurance claim.

How does a provider certify a DS-1 claim that was filed online?

When the claimant applies online, they receive a unique Form ID and give the provider printed instructions. The provider enters the TDI medical statement (M-01) on the Division's secure website with that Form ID and gets immediate confirmation, so no paper Part C is needed.

What dates does the DS-1 medical certificate need?

Part C asks for the first and most recent treatment dates and visit frequency, the date the patient became unable to do regular work, the recovery date or an estimated return-to-work date, and any emergency room, hospitalization or surgery dates. The instructions call for exact dates, never 'present' or 'current'.
