Washington Paid Leave Designated Representative Form

Washington Paid Family and Medical Leave lets a health care provider designate an authorized representative for a patient who cannot handle the program's administrative steps or name a representative themselves. The provider attests that the designation is in the patient's best interest. The form is not posted online; ESD's Paid Leave Customer Care Team supplies it for each case.

Who completes it

The patient's health care provider completes the designation and attestation; the patient does not complete a section because they are unable to act for themselves.

Submitting and payment

How to submit it
The provider requests the form from ESD's Paid Leave Customer Care Team and returns the completed form to that team.

Good to know

  • The provider attests three things: the patient cannot complete the Paid Leave administrative requirements, cannot designate a representative, and the provider is acting in the patient's best interest.
  • The process rests on RCW 50A.05.010 and WAC 192-800-150, which ESD cites on its health care provider page.
  • There is no downloadable PDF, so the provider contacts ESD's Customer Care Team by phone to start the process.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When can a Washington provider designate an authorized representative?

When the patient is incapable of completing Paid Family and Medical Leave administrative requirements and of designating an authorized representative on their own. The provider must attest to both facts and that the designation is in the patient's best interest.

Where does a provider get the Designated Authorized Representative form?

From ESD's Paid Leave Customer Care Team. ESD does not post the form on its website; its health care provider page directs providers to call the Customer Care Team, which supplies the form for the specific patient.
