Washington Paid Leave Family Leave Certification

Washington Paid Family and Medical Leave applicants use the Family Leave Certification to take paid leave caring for a family member with a serious health condition. The family member's provider briefly describes the condition and gives start and end dates for the leave needed. The applicant uploads it to their Paid Leave account so ESD can decide the family leave claim.

Who completes it

The applicant completes Section 1 with name, date of birth and Paid Leave Customer ID; the family member's authorized health care provider completes Section 2 and signs.

Submitting and payment

How to submit it
The provider returns it to the patient or family member within 7 calendar days of receiving it, and the applicant uploads it to their Paid Leave account or faxes it to ESD.
Who pays
No fee may be charged: ESD states that providers cannot charge a fee for completing the form.

Good to know

  • Specific dates are required: 'unknown' or 'indeterminate' will not do. ESD says conditions lasting more than a year can use a date one year ahead as the end date.
  • The provider signs under penalty of perjury that the condition meets the serious health condition definition in RCW 50A.05.010.
  • ESD accepts an FMLA form such as the WH-380-F, or a provider note, if it includes the information WAC 192-610-020 requires.
  • Corrections are made by crossing out, rewriting and initialing; ESD does not accept forms with whiteout. The bilingual English and Spanish form was updated June 2024.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

How long does a Washington provider have to complete the Family Leave Certification?

ESD asks providers to return the completed form to the patient or family member within 7 calendar days after receiving it. The applicant then uploads it to their Paid Leave account or faxes it to ESD.

What end date should be used for a long-term condition?

ESD's provider guidance says that when a serious health condition is expected to last more than a year, the provider enters a date one year in the future as the end date. Terms like 'unknown' or 'indeterminate' are not sufficient.

Does the applicant have to use all their family leave at once?

No. ESD notes that patients and family members do not have to use all their leave at once. The provider certifies the start and end dates of the need for leave, and ESD says the amount of time off is based on medical need.
