Unum CL-1329-OR Paid Leave Oregon Certification

Oregon employers with a Unum-insured equivalent plan provide Paid Leave Oregon benefits outside the state program, and their employees' serious health conditions are certified on this eight-page packet. The provider of the employee or of a family member answers pages 6 and 7: onset, duration, job relation, effect on work, the condition category and an estimate of continuous, intermittent or reduced-schedule leave.

Who completes it

The employee and employer complete pages 1 to 5 and the top of page 6. The health care provider of the employee or family member answers questions 1 to 7 on pages 6 and 7 and signs with printed name, practice type, phone and fax.

Submitting and payment

How to submit it
The completed packet is faxed or mailed to Unum's Benefits Center, as printed on each page. The employee's complete application is due within 90 calendar days from the start of leave.

Good to know

  • A reduced schedule under Paid Leave Oregon must be taken in full-day increments, so question 7 asks only how many days per week the employee can work.
  • The category list in question 6 includes Organ and Tissue Donation, covering preoperative or diagnostic services, surgery, postoperative treatment and recovery.
  • Incapacity here means being unable to perform at least one essential job function, attend school or perform daily activities for more than three consecutive calendar days.
  • A bilingual English and Spanish edition, CL-1329-OR-BL, is listed alongside the English form. Unum pairs the packet with its CL-1088-PFL authorization. The edition is dated 05/26.
