Oregon WCD Form 3531: Supplemental Disability

Oregon workers who held more than one job when injured can receive supplemental disability, wage replacement for secondary jobs beyond the temporary disability paid for the job where the injury occurred. To keep receiving it, the worker brings this form to the physician. The physician reports whether the worker is medically stationary, authorizes regular, modified or no work for set dates, and states which job the restrictions apply to.

Who completes it

The worker completes the top section with claim details and the primary insurer. The physician completes the physician section, including restrictions and whether they apply to the primary job, the secondary job or both, and signs it.

Submitting and payment

How to submit it
The worker is responsible for getting the form completed and returns it to the insurer.
Who pays
The form does not address payment for the physician's section.

Good to know

  • The form defines the primary job as the job where the injury occurred and a secondary job as any other job held at the time of injury.
  • Mark the medically stationary status with a date, or an anticipated date if the worker is not yet medically stationary.
  • Work authorization uses date ranges: a regular work start date, modified work from and through, and no work from and through.
  • A related form, 3530, is the Supplemental Disability Election Notification; Form 3531 is the physician's authorization of supplemental disability.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What is supplemental disability in Oregon?

Form 3531 defines it as wage loss replacement for the worker's secondary job or jobs that exceeds the temporary disability paid for the primary job, the job where the injury happened. Temporary disability alone replaces only the primary job's wages, so a worker with two jobs needs the physician's authorization on this form to keep the supplement.

Who is responsible for getting Form 3531 completed?

The worker. The form tells the worker they are responsible for having their physician complete it to continue receiving supplemental disability. The worker fills in the top section, the physician completes and signs the rest, and the form goes to the insurer.
