PGE Form 143034: Emergency Medical Certificate (Oregon)

Portland General Electric's medical certificate program uses Form 143034, which Oregon rule OAR 860-021-0410 says only a licensed physician, nurse practitioner or physician assistant may complete and sign. The clinician documents the diagnosed condition, the electrical equipment or services needed, and how losing them would significantly endanger the patient's physical health. Enrolled customers get outage outreach, backup planning help and more flexible payment arrangements.

Who completes it

The customer enters name, address, PGE account number and phone. Sections A to H are completed by the licensed medical professional: credential, patient name and relationship to the customer, diagnosis, severity choice, equipment or services, how loss would endanger health, and name, license, signature, phone and date.

Submitting and payment

How to submit it
Fax, mail or email the completed certificate to PGE's Medical Certificates team; customers can also start enrollment online or by phone.
Who pays
The certificate lists no fee for the clinician, and the program gives no discount on electric service.

Good to know

  • Section E has two choices: loss of power would cause or aggravate a serious illness without immediate danger to life, or the patient needs immediate response from emergency personnel with life-supporting equipment.
  • Under the Oregon rule, PGE is permitted to verify the accuracy of the certificate.
  • Enrollment does not prevent disconnection when a customer neither pays nor makes arrangements, and it does not give priority restoration.
  • The PDF has the English certificate on page 1 and the Spanish Certificado Médico de Emergencia on page 2.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Who can sign PGE's Emergency Medical Certificate?

Only a qualified medical professional under OAR 860-021-0410: a licensed physician, nurse practitioner or physician assistant authorized to diagnose and treat. Section A asks the signer to check which of the three they are, and Section H records their name, title, license number and signature.

Who qualifies for PGE's medical certificate program?

PGE says anyone whose qualified medical provider can affirm a medical need for electricity qualifies, including customers using equipment such as oxygen, those who need power to refrigerate medication such as insulin, and customers with extreme health risks or mobility concerns.

Is PGE Form 143034 available in Spanish?

Yes. The same PDF includes the Spanish version, Certificado Médico de Emergencia, on page 2, with the same sections A through H for the licensed medical professional and the same submission options.
