FAA medical forms: certification, CACI worksheets and status reports

The airman medical application, the BasicMed checklist, CACI worksheets and the status summaries pilots ask their own doctors to complete, with who fills each.

FAA medical certificates are issued by Aviation Medical Examiners. The pilot completes Items 1-20 of FAA Form 8500-8 in MedXPress, and the AME enters the exam in AMCS, then issues the certificate or defers. Treating clinicians get the rest: the Report of Eye Evaluation (FAA Form 8500-7), the Ophthalmological Evaluation for Glaucoma (FAA Form 8500-14) and status reports for sleep apnea, diabetes and weight-loss medication. BasicMed pilots can have any state-licensed physician complete FAA Form 8700-2.

This page lists 66 forms. CACI worksheets let an AME issue a certificate for a stable condition, such as hypertension, asthma or migraine, from the treating physician's Detailed Clinical Progress Note dated within 90 days of the exam. Most status reports go back to the pilot, who brings them to the AME for upload. HIMS checklists are for HIMS-trained AMEs. Vero Documents fills PDF forms from the visit note and the chart, and the clinician reviews and signs.

Frequently asked questions

Can my own doctor do a BasicMed exam?

Yes. Any state-licensed physician (MD or DO) can complete the Comprehensive Medical Examination Checklist, FAA Form 8700-2, and no AME designation is needed. The pilot fills Section 2, and the physician completes the exam items and signs the declaration in Section 3. The form isn't sent to the FAA: the pilot keeps it with the logbook. The exam is due every 48 months, plus an online course every 24 months.

What is a CACI worksheet?

CACI stands for Conditions AMEs Can Issue. Each worksheet is an AME checklist for one condition, from arthritis and asthma to hypothyroidism and kidney stones, checked against a Detailed Clinical Progress Note from the treating clinician dated within 90 days. If every item is met, the AME issues the certificate, notes 'CACI qualified' in Block 60 and keeps the documents. If not, the AME defers and sends them to AMCD or the Regional Flight Surgeon.

What should a clinical progress note for an FAA medical include?

The FAA's Detailed Clinical Progress Note sheet lists what it must cover: history, medications, exam, tests, assessment and plan. You write it on your own letterhead or in your own records. It goes back to the pilot, who brings it to the AME for upload or sends it to AMCD or the Regional Flight Surgeon as directed. The FAA warns that patient-portal notes and After Visit Summaries usually don't qualify.

Who completes FAA Form 8500-7?

An ophthalmologist or optometrist. The Report of Eye Evaluation covers history, phorias, fusion, pupils, fields, slit-lamp and fundus findings, acuity, intraocular pressure and the prescription in items 1-17. The eye specialist gives it to the pilot for the AME, or sends it to AMCD or the Regional Flight Surgeon when they ask for it. Glaucoma has its own form, FAA Form 8500-14, completed by an ophthalmologist.
